In May 2024, the City of Greeley Homeless Outreach Team met Tracy—just one month after launching. From the start, she stood out: expressive, fearless, and deeply aware of her worth.


This year, Tracy moved into her new home at StarRise Apartments, a fully furnished apartment with supportive services from North Range Behavioral Health. It wasn’t easy—she wasn’t selected in the first round of referrals. But thanks to community referrals and a strong network of support, Tracy’s persistence paid off.


Her success is the result of true partnership:
•    CORE Team (Greeley Police Department) built trust through consistent outreach.
•    SQUAD 1 (Greeley Fire Department) addressed medical barriers in real time, meeting Tracy where she was.
•    Homeless Outreach Team walked alongside her every step—helping with documents, appointments, and basic needs.


Today, Tracy is thriving in her beautifully decorated apartment, filled with Christmas cheer. Yet her heart longs for Jude, her beloved white Chihuahua who recently went missing. If you see Jude, please contact the Weld County Humane Society to help reunite them.
