Support for those experiencing homelessness

Outreach, Rehousing, and Permanent Housing Support

If you are experiencing homelessness, need help with rent or other services or are looking for resource information, contact HNC@unitedway-weld.org or call 2-1-1. Greeley's Housing Navigation Center is behind DriveWise on 8th Avenue.

Greeley’s Homeless Solutions Department consists of the Outreach Team and two housing support teams – the Rapid Rehousing and Permanent Supportive Housing Teams. The Outreach Team builds rapport and quickly works to help connect individuals to available resources and aims to meet people where they are and their immediate needs.

The Rapid Rehousing Team places more moderate-need individuals and families experiencing homelessness into safe, stable housing quickly. Through additional services, including case management, transportation, access to health care, food, employment assistance, and more, this team helps them maintain housing stability with support for up to 2 years with the goal of working toward independence and long-term housing.

The Permanent Supportive Housing Team provides voucher-supported permanent housing solutions and supportive services to access transportation, health care, food, and income assistance if qualified. The goal of this team is to help the highest-need individuals experiencing chronic homelessness obtain and maintain stable housing with ongoing case management support.

Definitions for Homeless Solutions Support

Support to help people who are homeless gain stability.

A person or family who lacks fixed, regular, and adequate nighttime housing.

Someone who has a disabling condition and has been homeless for over a year. Someone with a disabling condition who has had four or more episodes of homelessness in the past three years. Someone with a disabling condition who has experienced homelessness for over a year.

A database that homeless service providers use to track and manage resources.

A diagnosable substance abuse disorder, mental illness, developmental disability, or chronic physical illness. Or the existence of two or more of these conditions.

Government subsidized housing support. There are various levels of these vouchers.

A housing voucher for people who meet the criteria that provides financial help with case management services.

A yearly snapshot of the number of homeless people in a given city, county, or region. Volunteers conduct this survey count on one night in February each year. The survey is voluntary.

Homeless people living in emergency shelters or transitional or temporary housing.

People and families sleeping in a place not designated for or used as a regular sleeping accommodation.

Federal agency that sets rules and regulations, and provides guidance and funding for state and local governments, nonprofits, and for-profit businesses. HUD can provide grant money to homeless service resource providers.

A regional group that coordinates housing resources and service providers.

A system used by CoC and service providers to connect homeless people to housing and services.

A Colorado state division of DOLA that sets rules and provides guidance and money for local governments, nonprofits, and businesses for housing and homelessness. DOH can provide grant money to homeless service resource providers.
