Parents/Guardians of Minors
If you are the parent/guardian of a child under that age of 18 that will be employed by the City of Greeley, please CLICK HERE to complete the Authorization for Treatment and for Drug and Alcohol Testing of Minor form. This form must be completed and summited PRIOR to the minor submitting to pre-employment screenings.
